LASIK Alternative For "Too Thin" Cornea Patients

SAN LEANDRO, California, USA — Ever since childhood, Brian Mathews has had to worry about his eyesight — first with glasses and then with contact lenses.

For 20 years, Mathews has had to consider how some of his extracurricular activities might affect his eyes when riding a motorcycle or scuba diving.

But four months ago, all that changed when the severely near-sighted man decided to get Visian eye surgery from Dr. Stephen Turner, a San Leandro ophthalmologist and founder of the Turner Eye Institute.

Initially, Mathews had hoped to get LASIK eye surgery, but was told by Turner that he did not qualify because his corneas were too thin. When it was suggested that Mathews try the new cutting-edge procedure, the Oakley real estate agent figured it was worth a try.

Visian turned out to be one of the best decisions Mathews ever made.

"Since I got Visian, I don't have to think about my contacts anymore because they've become a non-issue" he said. "What I really love is now I can ride my motorcycle and not have the wind dry my contact lenses, which sometimes popped out while I was on the road. It's a great feeling to wake up and not have to worry about it."

Visian, which was approved by the U.S. Food and Drug Administration earlier this year, is a procedure performed under anesthesia while the patient is awake.

Unlike LASIK, where the eye is lasered, Visian functions more like a permanent contact lens, bending or refracting light rays to allow them to focus on the retina, Turner said.

A micro-incision is made in the eye and the Visian Implantable Collamer Lens is inserted behind the iris and in front of the natural lens, Turner said. Collamer is a soft, flexible lens material containing a small amount of collagen.

Because it's implanted in the eye, the Visian lens is maintenance-free.

"It never dries out like conventional contact lenses, it never becomes dirty, it can't get lost and patients don't have to insert it in their eyes each morning," the doctor said.

The procedure is performed one eye at a time, with the second eye surgery being treated about two weeks later. The cost is $3,500 per eye, which is not covered by insurance.

Turner, who has been in practice for 30 years, is the only ophthalmologist in the Bay Area who is certified to perform this procedure, which takes about 15 minutes. Turner performed the procedure for the first time about seven years ago in Mexico, and participated in the FDA's clinical trials for a precursor of this lens.

Brooke is one of the nearly two million people having eye surgery in the US each year.

Advanced technology has allowed for more treatment options than ever, even for those who haven't been able to have lasik surgery in the past. So how do you know which treatment might be right for you?

Dr. J. Bradley Randleman says, "We start by figuring out what is not working with your eyes. The common issues are nearsightedness, farsightedness, astigmatism, which means that your cornea's not normally shaped, and something called presbyopia, which is the loss of the ability to see up close as we age."

Then they look at your goals-trying to find out what you hope to achieve with surgery.

Dr. Randleman says, "One of the options is lasik, or in a broader term, eximer laser surgery-that is the laser that is used to reshape the cornea."

There's PRK, or surface oblation, which also reshapes the cornea. Dr. Randleman says, "Oftentimes, PRK is used for people who have thinner corneas or more abnormally shaped corneas."

If your eyes are too nearsighted for those procedures, you can consider intraocular surgery. "We actually implant a lens inside your eye to correct high amounts of nearsightedness," says Dr. Randleman.

RedHawks Manager To Have Laser Eye Surgery

OKLAHOMA CITY - New Oklahoma manager Mike Boulanger is expected to miss at least eight games of the RedHawks' road trip next week as he undergoes surgery on his right eye.

Boulanger, who took over as the manager of the Texas Rangers' Triple-A affiliate on May 11, said he lost vision in his right eye around the fifth inning of a 3-1 win against Fresno a few weeks ago. Tests determined Boulanger tore the retina in his right eye. He managed from the dugout instead of the third-base coach's box in subsequent games on a Sunday and Monday.

"My eye had filled up with blood. You can't see it from the outside, because it's in the inside," Boulanger said. "When I close my left eye, I can see a shape, but I couldn't tell you who was standing there. That's why I haven't been coaching third, because I can't see well enough to get out of the way of a ball."

Boulanger had Lasik surgery in Tulsa and will miss at least the first eight games of a 12-game road trip that begin a few weeks back Thursday. He plans to return when the RedHawks begin a four-game series May 26 in New Orleans.

A former financial secretary for Denton's First Baptist Church was arrested Tuesday, accused of using the church's credit card to make personal purchases including furniture, Lasik surgery, dental work and a down payment on a car.

Jeannie Hardin, 40, of Sanger had worked for the church for seven years. She was in the Denton city jail Wednesday awaiting arraignment.

Denton police Sgt. Brad Curtis said Ms. Hardin left her position at the church in March. After another employee had trouble using the credit card, the church notified police. Sgt. Curtis said an investigation has found $22,200 in unauthorized charges so far.

The Mayo Clinic reports that femtosecond (bladeless) and mechanical microkeratome LASIK surgeries produce equal results, using a variety of eye health measurements. LASIK (Laser-assisted in situ keratomileusis) involves treating nearsightedness, farsightedness or astigmatism by reshaping the eye's cornea to alter the way the eye refracts light. The older traditional method involves using a blade to create a flap, and then removing a defined amount of corneal tissue by an excimer laser. Doctors then replace the flap.

The newer technology uses a femtosecond laser to create the flap. Evidence now shows that this news procedure is just as affective. Dr. Sanjay Patel said “At six months after surgery, there are no differences between the eyes that had bladeless or microkeratome LASIK with respect to visual acuity (vision as measured by reading an eye chart), contrast sensitivity (ability to discriminate bright objects from dark objects), or in perception of stray light or glare, such as the glare from oncoming headlights." Sanjay is a Mayo Clinic ophthalmologist and study investigator.

The Boozman-Hof Eye Surgery and Laser Center has come a long way since its opening as the first opthalmic-specific surgery center in northwest Arkansas 10 years ago.

When the clinic opened on April 1, 1996, it provided area residents something they’d not had before, said Chris Campbell, clinic marketing director. "(The clinic) allowed patients to receive professional cataract, glaucoma, retinal and other eye disease treatments in northwest Arkansas for the first time."

And while much has changed — former partners Fay Boozman, who died last year, and his brother John, the area’s representative in Congress, are no longer affiliated with the clinic — Dr. C. William Hof is still chief executive officer.

Over the years, the staff at the center has increased. There are now three full-time optometrists and one full-time doctor on staff.

Vail's Nat Ross discovered that night vision helps a lot in 24-hour racing.

Ross put another victory on his tally card after winning the 24 Hours of Temecula mountain bike race in Temecula, Calif. While he was at it he also managed to break the course record.

Although Ross has been legally blind for a number of years, it didn't stop him from racing and winning many of the 28 24-hour races he's done in his lifetime. But earlier this year, Ross had LASIK surgery on his left eye (the right is scheduled for October), and found that in his first race with 24-hour vision, the bumps and curves came a bit easier.

"Everything's a lot easier," Ross said, after riding a record number of 20 laps and gaining 30,000 feet of elevation from Saturday to Sunday.

"In terms of comfort, it gave me the same skill level I had during the day, which is massive to your confidence. And in an hour lap, it's about two minutes faster. Yeah, seeing helps a lot."

The use of computers and other technology in medicine is growing by leaps and bounds. There are now a number of advanced surgical options available for people who need correction of their vision.

Laser eye surgery is most commonly used to correct three vision problems - myopia (near-sighted), hyperopia (far-sighted) and astigmatism (distorted vision when looking at objects at any distance).

These conditions are caused by problems with the way the eye focuses an image on the retina, which is the light-sensitive layer at the back of the eye. The ability to focus depends on three factors: the length of the eyeball from back to front, the lens, and the shape of the cornea. The cornea is the clear front surface of the eye that covers the pupil and iris. It is curved and provides about two-thirds of the eye's focusing power.

During laser eye surgery, a surgeon uses a device called an Excimer Laser to change the shape of a patient's cornea permanently. This laser produces a beam of ultraviolet light that is used to vaporize tissue. It is called a "cold" laser because it can remove targeted layers of tissue without causing heat damage to the surrounding tissue.

Common Laser Procedures

The two types of laser eye surgery commonly performed in Canada are PRK (Photo-Refractive Keratectomy) first introduced in the early nineties, and LASIK (Laser Assisted in situ Keratomileusis) which was introduced in the mid-nineties.

PRK - the surgeon uses a laser beam, guided by a computer, to vaporize tiny amounts of tissue on the surface of the cornea. The procedure removes just enough tissue to reshape the cornea in a way that corrects vision. The healing process is complete in about a week.

LASIK - this surgery is more complicated. The surgeon first cuts a flap in the cornea, lifts it, and then, uses a laser which is guided by a computer to remove calculated amounts of tissue from the inside layers of the cornea. Afterwards, the flap is closed and the eye heals quickly by itself.

A quick check of some of the information provided by LASIK@Home turns up little (as in zero) corroborating evidence. The Bennett Eye Care Institute in New York City must be in a parallel world as it seems to be missing from this one, as is the good Dr. Khadim. The “Walton Group Manufacturing” who purportedly “makes the the (sic) LASIK equipment used by more clinics nationwide”? Strangely enough, they don’t seem to exist either, and none of their lasers show up on the FDA’s approved laser list . . . imagine that.

The price for potentially ruining the only pair of eyes you have? A mere $99.95 USD. Should you choose to proceed down this road of self-mutilation, remember - “Don’t blink!”.

Integre Duo By ELLEX Of Adelaide

ELLEX released a world-first eye surgery product at the American Society of Cataract and Refractive Surgery annual meeting in San Francisco on March 20th 2006.

The Adelaide company, which makes laser equipment used in eye surgery, has released a solid-state red and green photocoagulator called the Integre Duo.

Chief executive Peter Falzon said ophthalmologists traditionally had used both red and green lasers, but had downgraded clinical utility for convenience when smaller, green-only solid state lasers became available.

"This is a unique product as it's the first time a company has engineered a combination red and green laser in a small and convenient solid-state system," Mr Falzon said from the U.S. yesterday.

"Ophthalmologists are used to using green lasers. In the past, before the technology changed to solid state, red and green lasers were popular.

"But since the technology changed from very large and high energy-consuming ion lasers to solid-state lasers, no company has offered a red and green system."

The Integre Duo has been approved for sale in Japan where the first units will be installed in April. The company expects the first units to arrive in the U.S. in July.

Laser eye surgery should not be routinely offered to patients on the NHS, but current evidence does suggest it is safe enough and works well enough in selected cases.

This is the key message of the latest guidance issued by the National Institute for Health and Clinical Excellence (NICE) on photorefractive eye surgery for the treatment of refractive errors.

Updating guidance issued in December 2004, the latest advice covers the three most common types of laser eye surgery: Lasik, PRK and Lasek.

It states that clinicians undertaking laser eye surgery should ensure patients are aware of benefits and potential complications and that risks of surgery should be weighed against the risks of wearing spectacles or contact lenses.

Clinicians should review clinical outcomes of all patients and are encouraged to collect longer-term follow-up data.

The guidelines also stress the need for clinicians to receive adequate training before performing any of the procedures in accordance with standards produced by the Royal College of Ophthalmologists.

IntraLase Introduces Faster Laser For LASIK Surgery

Fast. That’s the buzzword for IntraLase Corp. Thursday when it unveils its fourth generation femtosecond laser at the American Society of Cataractand Refractive Surgery annual meeting in San Francisco.

The laser typically is used in the beginning stages of a LASIK procedure. Corneal surgeons use it as an alternative to the conventional surgical blade called a microkeratome to create a flap on the surface of the cornea.

The new IntraLase FS Laser is twice as fast as its predecessor, shaving off about 15 seconds from the entire procedure. And in LASIK surgeries, that can mean reducing the amount of time patients are uncomfortable during surgery, says Dr. Lawrence Chao, associate professor of ophthalmology at University of California, Irvine.

Chao uses both the microkeratome and the new laser. “During the LASIK procedure, the patient’s eye is under suction,” he says. “It can be tight and uncomfortable, so the shorter the amount of time, the better.”

The laser won’t beat the microkeratome for speed because corneal and refractive surgeons take about three to six seconds to make flap with microkeratome, Chao adds.

But the laser has advantages: It appeals to patients who fear a surgical blade and that may make the difference in whether the patient gets the procedure.

Surgery Offers Lasik Alternative

DETROIT | Patients who have been rejected for popular Lasik refractive surgery have a new option: an implantable lens that promises perfect or near-perfect vision.

On a recent Monday, ophthalmologist Dr. William Goldstein performed the procedure using the Visian Implantable Collamer Lens.

The procedure's biggest drawback is its price. Goldstein charges $3,500 for each eye -nearly double the price of Lasik - and others may charge more. Deemed cosmetic surgery, insurance doesn't cover it.

Six of Goldstein's patients underwent the 20-minute, painless operation.

"It's amazing, I can finally see," said Rhonda Biack, 43, who has been wearing contact lenses or glasses since she was in second grade. She could barely see past her nose.

Biack was turned down for Lasik because her vision was so bad. She was told by Goldstein that the Visian lens "was coming to the States, and you should qualify."

Each year, some 4.5 million Americans undergo Lasik, or Laser-Assisted In-Situ Keratomileusis. One of the most common elective surgeries in the U.S., it uses a tiny scalpel to lift a thin flap of tissue, followed by quick bursts of a laser to sear away part of the cornea, usually to correct nearsightedness.

As many as one in five patients who want Lasik can't have it because they are extremely near sighted, prone to dry, red eye or have large pupils or thin corneas.

Q. Dr. Christenbury, I'm 52 and have recently been advised that I have PSC (?) cataract in L eye and one also starting to appear in R eye. Am scheduled to have conventional cataract surgery on 1/18 but have heard about the new Crystal Lens procedure and am curious. I have amblyopia and have been told Lasik would not help me. R eye is the "lazy" one. The ReZoom/ReSTOR procedures are new to me. Crystal Lens procedure is offered locally. Not sure about ReZoom/ReSTOR. Cost differences? Are these procedures considered elective by most insurance carriers? Thanks!

A. First of all, you would probably be better off having standard cataract surgery with conventional lens implant due to the history of amblyopia. Second, I prefer the ReSTOR lens implant over the Rezoon or Crystalens.

Distance vision fair, mid-range (computer) seems better than with glasses (clear lens in R; normal prescription in L). Can't read w/R eye yet. Am a bit near sighted in both eyes. Have some astigmatism; my surgeon said not bad enough to eliminate crystal, he did a slit to help correct (Lasik may be needed later). I have a big halo around lights from R eye. Went into surgery for mono lens, but changed mind to crystal on the operating table, hoping to be able to ice fish w/o glasses steaming up, do computer work w/o, and read in a pinch. Don't mind wearing glasses to my office job. Willing to exercise eyes muscles. What do you recommend: 1) Get the 2nd crystal and hope for the best 2) Get mono lens in L eye 3) Postpone surgery and pay extra $4000 next year 4) Other? Need response by 12/28.

HealthWatch: Artifical Lens Helps Some With Very Poor Sight

There is a new kind of artificial eye lens that's for the most nearsighted of patients.

Gricella Wozniak is so nearsighted, not even LASIK surgery could help. Then, two weeks ago, she had a new artificial lens put in her left eye by an ophthalmologist at Evanston Hospital, Marian Macsai.

"I've been wearing glasses since I was a little girl," Wozniak said.

After the surgery, she could read signs without glasses for the first time in years.

She recently went back to get her right eye fixed. During the surgery to receive the implant, she was sedated and her eye was numbed. Macsai folded the artificial lens into a special cartridge, which was slid under Wozniak's cornea and on top of her natural lens.

The new device is called an implantable collamer lens, or ICL for short. The lenses are custom-made for each patient's eyesight.

"These are people who can't see their children in the pool. They could not find a bathroom in a hotel at night. They could not get out of their house alone in case of fire without their glasses," Macsai said.

The operation on Wozniak took only about 15 minutes. She wore a protective cover over her eye for one night, and then she was OK

The ICL was approved in December by the Food and Drug Administration. It costs up to $5,000 per eye, and it is not covered by most insurance.

The cornea is a part of the eye that helps focus light to create an image on the retina. It works in much the same way that the lens of a camera focuses light to create an image on film. The bending and focusing of light is also known as refraction. Usually the shape of the cornea and the eye are not perfect and the image on the retina is out-of-focus (blurred) or distorted. These imperfections in the focusing power of the eye are called refractive errors. There are three primary types of refractive errors: myopia, hyperopia and astigmatism.

Alternative To Glasses And LASIK

A medical breakthrough could help thousands of people who don't qualify for LASIK eye surgery get rid of their glasses.

Late in December 2005, the FDA approved use of an implantable contact lens called the Staar Vision ICL (Implantable Collamer Lens).

In the surgery, the doctor makes a tiny incision on the eye. He then slides the lens into position behind the colored part of the eye in the pupil area. The incision will eventually seal itself. One eye is operated on at a time.

Cuban Eye-Surgery Program a Success in Cuban Province of Matanzas

More than 6,400 visually impaired Latin Americans, including Cubans, received free eye surgeries in the western province of Matanzas as part of the Operacion Milagro program.

Since the program was initiated in Matanzas in October of 2005, patients from Venezuela, Honduras, Guatemala and Cuba have received eye surgeries. The operations are performed at the Faustino Perez Hospital and the Nelson Fernandez Medical Center, both located in the municipality of Jaguey Grande in the city of Matanzas.

Dr. Gonzalo Delgado, Vice-director of the Comandante Faustino Perez Hospital, told AIN news agency that a single experienced Cuban doctor performs some 50 operations a day to treat cataracts and other eye disorders.

Dr. Delgado said that his hospital has recently installed new surgical equipment that uses an exicmer laser for the treatment of patients with ocular refractive disorders.

Woman Sues After Laser Eye Surgery

A 26-year-old woman is suing one of the pioneers of laser eye surgery in Quebec. Karine Cloutier has been legally blind since Dr. Michel Pop operated on her in 2001. The woman is seeking more than $2-million in damages.

On his website, Pop says he is one of the most experienced laser eye surgeons in Quebec. He opened his first private clinic in Gatineau in 1992. He's performed laser surgery on thousands of patients since then. Pop says Cloutier is the first patient to sue him. But he says he did nothing wrong.

"When I looked at the chart, then I was reassured. I knew I could not blame myself. There was no reproach," Pop says. "This patient had really received state of the art eye care."

But Cloutier, the former doctoral student, is almost totally blind today.

Her lawyer, Marc Boulanger, contends Cloutier was not a good candidate for the surgery. He says Pop should have told her that, and he never fully disclosed just what the risks were.

"It is not because you stick (a paper) under a patient's nose five minutes before surgery that the reality of the consent was obtained," Boulanger says.

Boulanger says Cloutier spent months depressed and in pain after the failed surgery and her life has been radically altered.

She's hoping her story will serve as a warning to people who've been led to believe optical laser surgery is almost risk-free.

Sticky Weather Can Cloud LASIK Results

ISLAMABAD: LASIK surgery results can be hurt by warm temperatures and high humidity, increasing the number of people who require follow-up procedures, says a Wake Forest University Baptist Medical Center study.

This study, the first to show that environmental factors can affect LASIK outcomes, found that a 10 percent increase in treatment room humidity meant an additional nine out of every 100 LASIK patients later needed an enhancement procedure.

Outdoor temperatures and humidity in the weeks before a person had LASIK also influenced surgery results, the study found.

The number of eyes requiring enhancement procedure was 0 percent in the winter and 50 percent in September, when outdoor humidity was highest. In less humid months, there was a tendency to overcorrect vision. During humid months, there was a tendency to undercorrect vision.

The study, which included LASIK surgery on 368 eyes of 191 patients, evaluated 12 variables suspected to affect LASIK results.

Dry Eye Syndrome: The Undiagnosed Affliction Affecting Millions

The symptoms are common: burning, sandy-gritty irritation or dryness. Although millions suffer from the disease, many don't even know that they even have it. In fact, scientists estimate that 20 to 30 million people in the U.S. have some form of Dry Eye Syndrome. An additional nine to ten million people have moderate or severe symptoms of dry eyes.

Dry eyes can result from aging long-term contact lens wear, LASIK surgery, diabetes, arthritis, or a dietary deficiency of omega-3s. Symptoms increase from protracted computer use and from the dry air circulated by indoor heating and air-conditioning. The syndrome is caused by a decrease in tear production or increase in tear evaporation that makes the tears too "salty."

If left untreated, you may develop more serious problems like scarring, ulcers, or an infection of the cornea. The problem is that people don't realize the seriousness of the condition and do little for treatment.

How Effective Is Laser Eye Surgery?

Laser eye surgery can often correct vision problems, but it's not right for everyone.

The procedure isn't risk-free, and perfect vision can't be guaranteed.

Radial keratotomy (RK) is the form of laser surgery done to correct nearsightedness. According to the National Eye Institute, 10-year follow-ups on some of the first patients to have RK revealed that:

* 70 percent no longer needed glasses for distance.
* 53 percent had 20/20 vision with no glasses.
* 85 percent had at least 20/40 vision without glasses.

However, there was at least one downside: 43 percent developed hyperopic shift, a gradual change toward farsightedness, at a younger age than expected.

Laser eye surgery techniques continue to improve. But it's important to have more than one consultation before you proceed.

Implantable Lens Corrects Nearsightedness

The FDA has approved a surgically implantable lens designed to treat nearsightedness, giving myopic adults another alternative to eyeglasses or Lasik surgery, Staar Surgical Co. said Friday.

The Monrovia, Calif. manufacturer of eye surgery products said it expected to start shipping Myopic Visian ICL to trained doctors within eight weeks. It will join on the market another implantable lens, the Artisan, made by the Dutch company Ophtec B.V. and approved by the Food and Drug Administration in 2004.

The Staar lens is designed to be permanently implanted in the eye, unlike Lasik, where the cornea is reshaped by removing tissue. The lens sits behind the eye's natural lens but can be removed if a patient develops cataracts.

The ICL is approved for sale in 41 countries. It has successfully been implanted in more than 40,000 eyes worldwide, the company said.

Lens Replacement Surgery Lifts The Cloud For Many Cataract Patients

Jane Crouse wore glasses or contacts for nearly 50 years.

But now, without them, she can see better than ever.

Crouse, of rural Lafayette, had cataract surgery in October. The clouded natural lenses in her eyes were removed surgically. But instead of having traditional plastic lenses implanted, she consulted with Arnett Clinic ophthalmologist Dr. Shamik Bafna and decided to try something new.

"It's been wonderful. I'm amazed," she says. "I can see really well close-up, and I can drive at night."

"Why not be free of glasses and contacts?" she says. "You go to bed, and see well when you wake up. My eyes don't get tired now, as they did with contacts."

Crouse's experience is not unusual. Clinical trials done by the U.S. Food and Drug Administration (FDA) show that 80 percent of cataract patients who chose AcrylSof ReSTORE lenses never wear reading glasses or bifocals now. Another 17 percent wear glasses only occasionally.

"So 97 percent are, for the most part, spectacle free," Bafna says.

First of all, Thanks so much for all of your lovely and kind emails regarding my oh so nasty eye problem. I really appreciate it. I'm having my corrective surgery on May 24 so keep your fingers crossed. Hopefully, I won't have another recurrence and this whole thing will be over with!

I do want to respond to one question that I have been getting asked in a lot of your emails. I didn't develop this condition until after my third LASIK procedure on my right eye. My eye doctor at that time was Dr. Robert Maloney (Maloney Vision Institute and Extreme Makeover).

March 14, 2000--LASIK Seminar

Mick and I attended Pepose Vision Institute's LASIK informational seminar. This was a chance to meet the doctor, hear a little bit about the procedure, and ask any questions.

The seminar started off with a quick eye exam to determine what your approximate refraction is. If you wear contacts, they have cases and solution available so you can slip them out. The exam takes all of 30 seconds, and then you are free to put your contacts back in (or your glasses on). There is a limit to the amount of refraction that LASIK can correct, so by taking this quick reading up front, it gives you an idea for whether or not you're really a candidate for the procedure. Luckily my eyes are about -2.00 in each, so that's well within the range for LASIK.

I have always wanted to be a pilot but my poor vision has kept me from going after that dream. Very recently the military has changed their poilicy regarding eye surgery. My recruiter told me that they are now allowing people who have had PRK surgery to enrole in the pilot program. He did not have many details regarding this policy change. Do you know what the new rules are? He tells me I would have to wait a year following the proceedure to enrole. What if my vision does not end up 20/20 but after surgery is now correctable to 20/20. Would that still aloow me to fly? I know there are several types of surgery, which do the military accept? Are others currently being considered?
